Private Hire for NDIS participants:

$65 for 1 hour

Must be a NDIS participant - NO EXCEPTIONS. 
Family living under the same roof, support workers, and therapists are welcome to join the session with the NDIS participant. 
All families will be provided with a travel exemption letter.
Parents, carers. and therapists are required to wear masks throughout the session unless exempt.

My Puzzle House is classed under the Health Care and Social Assistance Sector as a Community based service which is required to ensure the health, safety and wellbeing of vulnerable people, which is why My Puzzle House are able to provide these services through this difficult time.

AT CHECKOUT PLEASE TYPE IN THE NOTES:
**NDIS Client Name Number as proof of being an NDIS participant.
**Plan manager details if you would like us to email the invoice directly to them for payment.

If you would like the reciept/invoice to be in your childs name, please put there name as the orderer.

Cancellation Policy: No refund if cancelled less than 24 hours prior to appointment. Cancellations MUST be made via phone on 03 5611 0404.

What are we doing to prevent the spread of COVID-19?
We are wearing masks.
We are social distancing.
We have a COVID-safe plan.
We have completed infectious control COVID-19 training.
We leave a minimum of 15 minutes between sessions for cleaning which includes sanitising all equipment & high touch areas.
Food and drinks provided as takeaway with disposable plates, bowls and cups.
We are following all guidelines as per the DHHS directives.
